Monthly Counseling
Attached is the sheet you will use for monthly counseling, as well as an example for your reference. Below are the instructions on how to fill one out, as per the XO’s orders.
Instructions
Typed in ALL CAPS, Courier New 10 pt font
Maintain 5 lines per section
At the end of a line, do not press the space bar to move to the next line. Push the down arrow and continue typing.
SNM = Said Named Midshipman and must be used correctly
At the bottom of the form, MIDN should hand print their names in ALL CAPS on the “I, _____________, certify…” and sign where appropriate.
